1. The service member’s first and last name
2. City and state of home-of-record (where the person enlisted)
3. Year(s) of service
4. Unit of assignment (such as company and platoon, if known)
5. Details of photograph (date, location, etc.)
6. Any other biographical information you may know
4. Enter your email address so that we can email you a confirmation.
5. Click on the “Upload” icon to submit your photo.
